If you are preparing for your first treatment session, you are in the right place! Please fill out an intake form and read the liability waiver if you would like. 

Please read and fill out our INTAKE FORM, and read the LIABILITY WAIVER, which you will sign in person at the beginning of your first treatment session.

Please notify us if you are having issues accessing any of the above files. Thank you!

• DO NOT prepare for your session by giving your pet a bath. 
• DO NOT give medications that have not been discussed or written on your intake form.
• DO NOT take your dog for a run or exercise them before or after your treatment session. 
• DO NOT use other therapy modalities (hydrotherapy, other electro stim therapies) without notifying your therapist prior. 
• DO NOT schedule a play date or exercise for after treatment sessions. 

• DO use a special leash or your treatment leash when taking your dog to its session, this tells them they are doing something positive, not exercising or going to the vet. 
• DO notify your therapist of any recent changes to health, or medications, or topical medications you have used that day.
• DO encourage your pet to see the treatment as a treat! They really do love it!
